Augmentin 375/125mg Amoxicillin/Clavulanate/ Mercaptopurine/Iron Oxide Anticoagulant Co-Amoxiclav:Co-Amoxiclav is contraindicated in the setting of infection with amoxicillin-sensitive bacteria or in combination with anticoagulants (blood thinners) due to possible interaction of the causative organism with the co-amoxiclav. If co-amoxiclav is the etiology of infection, the dosage and duration of treatment will be adapted depending on the type of amoxiclav. Doses of co-amoxiclav may be adjusted by the physician based on patient preference, risk of infection and severity of infection to be determined by the severity of the infection. In case of excessive or prolonged therapy, the physician may adjust the dosage according to the severity of the infection. If co-amoxiclav is the causative organism, the dosage of co-amoxiclav may be administered in divided doses on the basis of patient response to the specific infection. The duration of treatment and dosage of co-amoxiclav should be individualized according to the severity of the infection. In the setting of renal insufficiency (renal failure), the dosage of co-amoxiclav may be adjusted based on the renal function of the patient. For patients with severe renal insufficiency (creatinine clearance less than 30 ml/minute) and a baseline creatinine level of less than 10 mg/dL (ascorbic acid) or patients with a baseline creatinine level of 10 mg/dL or more, co-amoxiclav 375 mg/125 mg and amoxicillin-clavulanic acid (clavulanic acid) 750 mg/125 mg may be administered together.
